We are in the process of replacing our current records management system.  We ask that you help us
by filling out our new forms which will be handed out by our front desk and which can be downloaded from our "Downloads" page. 

The forms might look very similar to what you have filled out with us some months ago but they are not the same.  Our forms have been updated.  So, if you filled them out prior to May 10, 2010, you will need to fill them out again.  We apologize for the inconvenient this might represent.

Also, we will ask you for a photo ID which will then be associated with your medical records for security reasons.

This change will help our doctors access your health information more effectively, but more importantly protect you!
