2-   Bring the following documents: a) an updated photo ID, preferrably your driver's license, or a NY state issued ID card, and b) your latest health insurance card.  The information on these documents will not be shared with anyone unless needed for the services we will provide to you, this, according to our “Notice Of Privacy Practices” which is compliant with HIPPA laws.  Please click on the 'HIPPA" button above for said notice.
